Please read and sign all the required documents after booking your online appointments for dietitian services. Once confirmed, check your email from Simple Practice for the links to the forms and your appointment confirmation. Note that your session may be cancelled WITHOUT notice if these forms are not signed.

A valid credit card is required to secure your new client appointments for dietitian services. Your card will only be charged in the event of a missed appointment (no-show), a cancellation with less than 24 hours’ notice, a denied insurance claim, or to cover any applicable co-pays or co-insurance not paid at the time of service during your online appointments.

Please note that you are responsible for any co-pays or co-insurance costs that may apply, so it’s important to verify your insurance coverage ahead of time, especially for dietitian services. Being in-network does not automatically mean your visit will be covered by insurance. A referral may be required, particularly in California, but it does not guarantee that your insurance will cover the visit.
